Eligibility: Homes for the Aged (Long Term Care Facilities) -- persons 18 years and over who require assistance with daily living - for details, see separate local Community Care Access Centre entries
Toronto Supportive Housing Program -- elderly persons who require personal support and homemaking on a 24 hour intermittent basis
Adult Day Programs -- persons living in the community who are frail or socially isolated, or who have a cognitive impairment or dementia of aging

How to apply: Homes for the Aged (Long Term Care Facilities) -- admission process includes assessment of applicant's health, nursing and psychosocial needs

Service description: Homes for the Aged (Long Term Care Facilities) -- semiprivate or private accommodation
  • medical and nursing care
  • social work, rehabilitation, spiritual and religious care, complementary care, art and music therapy, recreation, housekeeping, maintenance services
  • dietary staff
  • rate reductions may be available for qualified applicants

    Short Stay Program -- at various facilities
  • admission for 7-30 day periods
  • short term admission to provide respite for caregivers, maximum 90 days per year
  • convalescent care for those who need to recover strength, maximum 60 days per year
  • includes persons with Alzheimer Disease at 10 facilities

Alzheimer Disease -- special care units for seniors with cognitive impairments or Alzheimer Disease at various facilities
Toronto Supportive Housing Program 416-392-8543 -- purchases personal support and homemaking services and 24 hour intermittent on-site supervision on behalf of clients in retirement homes and apartment buildings
