• Alzheimer day program
• Stroke Survivors day program

Full Circle 416-422-2026 ext 269 -- community psychogeriatric case management team for persons 60 years and over with mental health needs

• in partnership with Community Care East York and Toronto East General Hospital Community Outreach Services
