How to apply:needs that cannot be met on an outpatient basis a medical condition that can be adequately treated in the home or school
Placement in long term care homesadults 18 years and over with care needs that can be met in a long-term care home, who need nursing care, assistance with activities of daily living or on-site supervision or frequent monitoring
Self referral if referral by family, friends, health professionals or others, consent is required Placement in long term care homes:
Fees: Free with Ontario Health Insuranceeligible applicants will be placed on waiting list of one to five facilities in Ontario chosen by the applicant applicants will not be placed without their consent applicants may select individual facilities, but may not apply to them directly
Physical access: Wheelchair accessible building including main entrance and barrier free washrooms
Service description:Arranges in-home or school-based health and social services, and placement in long term care homes service coordination information and referral for related community services
In-home services:
nursing care physiotherapy, occupational therapy speech-language pathology social work nutritional counselling personal support, homemaking if eligible for above services, other services may be provided such as drug card, medical equipment or supplies, laboratory services, transportation
School-based services:professional services to enable children with disabilities or ongoing diseases to attend school
Acquired brain injuries:access to and coordination of services
Also information and referral assistance in linking to existing community support services, which may also be contacted directly by consumers such services include friendly visiting, meals on wheels, volunteer transportation, security checks, that are relatively low in cost and primarily volunteer-based also help finding other services, such as attendant care for persons with physical disabilities and supportive housing servicesPlacement in long term care homes:
Organization type: Non Profitaccess for admission to all long term care homes, or for transfer to facilities outside Scarborough does not include seniors apartments, retirement homes, or chronic care hospitals subsidies for ward accommodation may be available -- apply directly to facility after placement
Short stay:respite for caregivers includes persons with Alzheimer Disease Convalescent Care -- following surgery or illness medical and therapeutic supportive care provided in a long term care home usually 30-45 days
Crisis placements:when support suddenly breaks down at home, person is placed wherever a bed is available, while awaiting placement in the facility of choice
