Eligibility:
  • Homes for the Aged (Long Term Care Facilities) -- persons 18 years and over who require assistance with daily living - for details, see separate local Community Care Access Centre entries
  • Toronto Supportive Housing Program -- elderly persons who require personal support and homemaking on a 24 hour intermittent basis
  • Adult Day Programs -- persons living in the community who are frail or socially isolated, or who have a cognitive impairment or dementia of aging

How to apply:
  • Homes for the Aged (Long Term Care Facilities) -- admission process includes assessment of applicant's health, nursing and psychosocial needs
  • for information and to apply to long term care facilities, or to short stay program including crisis admissions, call local Community Care Access Centre (see separate entries)
  • Supportive Housing and other programs -- call Intake Unit 416-392-8545, fax 416-392-8457
  • Adult Day Programs -- see branch records

Service description:
  • Long Term Care Facilities
    • semiprivate or private accommodation
    • medical and nursing care
    • social work, rehabilitation, spiritual and religious care, complementary care, art and music therapy, recreation, housekeeping, maintenance services
    • dietary staff
    • rate reductions may be available for qualified applicants
    • volunteer opportunities

  • Short Stay Program -- at various facilities
    • short term admission to provide respite for caregivers, maximum 90 days per year
    • convalescent care for those who need to recover strength, maximum 60 days per year
    • includes persons with Alzheimer Disease at 10 facilities

  • Crisis Admissions
    • contact local Community Care Access Centre (see separate entries)

  • Alzheimer Disease
    • special care units for seniors with cognitive impairments or Alzheimer Disease at various facilities

  • Toronto Supportive Housing Program 416-392-8543
    • purchases personal support and homemaking services and 24 hour intermittent on-site supervision on behalf of clients in retirement homes and apartment buildings

    • rent and food costs, paid directly by the client
    • meals provided in a central dining room at 2 sites
